You agree to provide complete, accurate, and current information during registration, including health, medical, and emergency contact information. Withholding or providing inaccurate information may put you at risk and may result in suspension of services.
Participation in fitness, exercise, and adaptive training programs involves inherent risks. By registering, you confirm you have read and accepted the Assumption of Risk, Waiver of Liability, and Indemnity Agreement presented during registration.
By typing your name in the signature field, you provide your electronic signature, which has the same legal effect as a handwritten signature.
By providing your email address and phone number, you agree to receive communications related to your registration, scheduling, billing, and the services you request. You may opt out of non-essential communications at any time.
You agree not to misuse the site, attempt unauthorized access, submit false information, or use the service to harass others or violate any law.
The site and its content are provided on an "as is" and "as available" basis. We make no warranties, express or implied, regarding uninterrupted availability or fitness for a particular purpose.
To the fullest extent permitted by law, All Adaptiv and its affiliates are not liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the site or services.
We may update these Terms from time to time. Continued use of the site after changes are posted constitutes acceptance of the updated Terms.
